Health systems worldwide were designed under an assumption of climatic stability that no longer holds. As climate volatility becomes a structural rather than episodic feature of the operating environment, health infrastructure — staffing models, supply chains, facility planning, and clinical training — requires a corresponding redesign. This piece examines the nature of that mismatch and outlines the operational shifts required to close it.

The health impacts of climate change are frequently discussed in terms of populations broadly affected by heat, flooding, or disease vector shifts. Far less attention is paid to a specific, disproportionately exposed population: the smallholder farmers, agricultural laborers, and food producers whose daily working conditions place them at the direct point of climate impact — and whose health systems are often the least equipped to respond.

This article argues that the reverse sequencing is closer to correct: last-mile delivery is not the final step of a food-health system, it is the design constraint that should shape the system from the outset. It examines why last-mile failure is so persistent, and what a genuinely sustainable, last-mile-first food-health system requires.
